Initial Session Documentation

Below are the initial session packets. The intake packet includes HIPAA information, Informed Consent,  and Financial Policy information. Feel free to print out a copy, complete it, and bring it to the first scheduled visit. Otherwise, we will have the documents available for you upon your arrival. Please consider arriving 15 minutes earlier if you choose to complete your packet at the office.
